Kaweah Health Outpatient Diabetes Center
Outpatient Diabetes Center provides multi-disciplinary services for the adult population. The Diabetes Center offers education by certified educators to patients with a physician's referral. Services are based on the assessment of the patient's needs and may include instruction on diet, blood sugar monitoring, prevention of complications, insulin injections, insulin pumps, and continuous glucose monitoring. Registered dietitians offer diet counseling to adults as well as to the pediatric population.

Sierra View Medical Center

Tulare County Suicide Prevention Task Force
Tulare County Suicide Prevention Task Force (SPTF) functions as a multi-disciplinary collaborative, combining representatives from government, education, community-based service providers, and community members. Training, activities, and education are provided throughout community to increase awareness of the signs and symptoms of suicide risk as well as prevention and early intervention best practices.
